Family stories suggest that Matthew emigrated to the USA. In 1861 he was described as an Agricultural Laboorer and was living at the family pub in Yaxley. In 1871 there is a Matthew Lilley with his wife Fannie, daughters Fanny and Rosa and son Walter in Chorlton on Medlock, Manchester. This would fit well with the move of brothers Robert and John to Manchester in the 1870s after the death of David the elder in 1869. This 1871 Matthew is definitely the Matthew Lilley found in 1880 (and 1900) in New Haven, Connecticut, USA with wife "Annie" and daughters Fannie and Rosa. The tie between the Manchester and US Matthews is strengthened by the the fact that in 1871 we have a "Professor of Medicine" and in 1880 a "physician". The 1871 census form also has what looks like "MD USA" under the occupation. It seems hard to make the leap from 1861 Ag Lab to 1871 Professor of Medicine but the 1871 Matthew has birthplace Huntingdon and the previous censuses show no Matthew other than ours who would fit this age and birthplace. Is it possible that as second son he went to the USA in the 1860s and was trained as a battlefield surgeon in the Civil War?
The only Matthew Lilley marriage 1861 to 1865 is in Henley district in Sep 1864 and there is an Ann Robinson on the page. Henley is the registration district for Watlington, Oxfordshire given as birthplace for his daughter Fanny in 1871. According to the censuses she was born about 1862 or early 1863 (8 in March 1871 and 17 in 1880). There are no Fanny or Ann Lilley births registered in this period but there are some Ann Robinsons in the surrounding area (not Henley) and an interesting Worcester registration for a Fanny Robinson in Dec 1863. Worcester is the birthplace of Fanny (the elder) so it could be possible that she had a daughter in Watlington and registered her later in her home town. Maybe Matthew came back from the US the next year and married her.
